Asthma Action Plan Framework

Effective asthma control depends on caregiver education, early symptom recognition, and structured self-management. Central to this approach is the Asthma Action Plan, which uses a simple traffic-light “zone” system:

Prevention & Weekly Education

This framework, combined with prevention strategies such as allergy control, vaccination, sleep, nutrition, physical activity, hydration, and hygiene, empowers families to reduce exacerbations and improve daily quality of life. Weekly asthma education was provided.

Care Navigation & Follow-ups

To address both clinical and social drivers of asthma outcomes, our team developed a comprehensive, community-integrated program focused on education, early identification, linkage to care, home trigger assessments by our team, and PCP/pulmonology follow-ups.

On Community Outreach — We actively partnered across public, private, and non-profit sectors to maximize access and representation:

Active collaboration with YMCAs, Boys & Girls Clubs, and regional youth centers.

Community Advisory Council

Assembled and managed a dedicated Community Advisory Council for ongoing feedback and governance.

Activities included tabling events, health fairs, intake screenings, and direct navigation to care to ensure increased community participation and health equity.
